Aviva is proudly certified by the Ohio Department of Aging (ODA) to provide both basic assisted living services and memory care services. This certification affirms that our community meets all applicable state requirements for participation in the Assisted Living Waiver Program. We are currently accepting individual consumers who are enrolling in the Assisted Living Program and are an active participating provider in MyCare Ohio.
